I have arrived at Lisbon. Let me answer my own question first. At least for Fontecruz, the COVID test requirement is for checkin only. No new test is required during my entire stay of four nights or longer. If I check in to another hotel after my stay at Fontecruz, I will need a new test. I saw COVID test trailers at many places; so getting a test will not be a big deal. I do not know the cost, though.

Before I arrived at Lisbon, my SNA upgrade request for a suite was denied, but they gave me the best non-suite room with balcony as a Titanium member. I saved the SNAs. I really appreciate the free upgrade and love this room. Compared to Ivens, Fontecruz is about 10 extra minutes walk to the tourist areas. It is not a big deal for me. The hotel breakfast is very simple. Other than the cold items on the picture, you can order eggs, omelette, etc, plus cereal and dessert items.

The staff in this hotel is very friendly. I have no complaint at all. My wife thought that I made an excellent choice of hotel this time, compared to staying at Marriott Lisboa last time.
