You can only go to USA from EU if you fit into certain categories (i.e. resident there / national / certain visa categories) - not just transiting on a holiday. You could also do it if you spent 2 weeks outside the EU first in a neutral third country.

Yes - Madrid (Iberia), Frankfurt (Lufthansa).
KLM (Amsterdam), Edelweis (Zurich) and BA (London) all start (at the moment) end of October... I wouldn't rule out cancellations from those 3 though.
