If FA was concerned that would be the case, they could ask for a "physician or surgeon" then. But I don't think the FA is qualified to determine you're going to need coat hanger surgery, and I would think a dermatologist is better than a PhD of Chinese literature, which is the point of discussing requests for a "doctor".
