On the traffic lights: I´m not sure this is the right explanation, but occasionally you will see TWO red men at wide crossings where there is a divider in the middle of the road where a pedestrian can wait, while cars may be given a green arrow for right turns. So one half of the pedestrian crossing might have a green man while at the same time the other half might be red . So two red men might be seen as some sort of an extra warning.
