After a bit of back-and-forth communications, the miles were deposited. They came in two tranches, the first for 10,000 miles and the second, about 10 weeks later, for the remaining 15,000.
For bank and brokerage accounts, it is always necessary to mail in something with a signature on it. I always attach a copy of the miles offer with whatever I send in, with a note that opening the account is contingent on my receiving the miles offered. Also, I always make electronic copies of the online offer and my acceptance of it, either via copy and past to a word document, and/or screen shots. (This latter sentence applies to all miles offers of all types.)
Several times my documentation has made the difference between receiving and not receiving the miles.
