I once (in Charlston, SC) received a bill (I was with my husband in which the waiter who possessed of all the charm of a drink blender had written

"15% service charge NOT included".

Livid my husband wrote

"15% service NOT provided" and left 1c extra on the credit card form. Equally unknown he rang back and told the "manager" the same night....
