Stick with information backed up by paperwork. Whatever her name is on the divorce decree (or whatever it is called in your state) is the name you should supply. DOS is not asking you to go out and do research into whether she has changed her name and, if so, to what.

If it so happens that DOS asks you to justify the answer, you have an attested (hopefully) copy of the Decree handy.
