I would think that unless you're at larger stores, yes, using anything other than cash will be difficult.
As for exchanging money, are you talking about doing a cash advance on a Visa card by using it at an ATM? If so, unless I'm mistaken, you will be hit rather hard with cash advance fees. I always just withdraw money using my ATM card. Granted, my bank charges me a 3% fee for the international withdrawal, but I haven't found a better option...
