This is the part I'm most confused with. I've called Cathay many times asking for anything written to prove they have refunded the money, so I can ask AMEX to confirm. Cathay refused (my many requests) and basically declaring their words of the refund being processed on August 23rd is proof enough.
IMO you are overthinking it. File charge back with AMEX and get temp credit. AMEX will reach out to CX and if they don't respond, credit is permanent.
