Its a standard practice. What is the proof that you actually had a medical emergency. These justifications are needed for a multitude of purposes. If no proof was needed, everyone will just claim they had a medical emergency and waive fees.
Note - Similar notes are needed when booking a bereavement fare (10% discount)
