IIRC several TAs here mentioned doing it when necessary
id think it would be US PTS(CTS?) rather than US TAs?
US is by far largest number of cardholders, may be factor
id think limit would be by country of cardholder, not sure
is shorter stay only better for upgrade (entire stay) ?

If your TA books through FHR, the hotel pays commission to the TA.
With LHW, if your agent makes the booking, the hotel pays a commission to the TA.
kiwi/visa, preferred, SLH - also allow TAs to book
some of these websites have TA portals
IIRC FHR used to be bookable on RC/etc sites
