The activation via
https://www.myonlinecreditcard.com does not function as expected. The activation via the activation phone number on the card does function as expected. Despite repeated calls to customer service the card may not be activated.

These are the steps that I performed (received the card on Fri 12/30), set up the account online, and the card worked after the phone activation. I used it to pay a hospital bill, and the payment went through successfully, so at this point, just waiting for the 60K miles to show up...
