I had a similar issue - something about a middle initial being on my card but not my skymiles account. AMEX pointed the finger at Delta. Delta pointed the finger at AMEX. They couldn't link the accounts. I ended up cancelling all of my amex's after wasting hours on the phone with them trying to fix it - such a CF.
