You applied for cards from no other banks in the 24 months?

You have to count every card on your credit report opened in the last 24 months, not just ones from chase.

And don't count inquiries, but actual cards. Look at the "opened on" date for each card on your credit report, and count the ones that are in the past 24 months. (Then if you opened any Chase cards in the last 24 months that don't show up on your credit report, either because they're too new or because they're business cards, add those to the count too.)
