I think it depends on the hotel itself, i.e. when they submit the 'paperwork' to booking.com
I have stayed in some, several times that consistently post within one or two days and one in particular (again three times) that always posts after the maximum 28 days.
In answer to your second question, I have had one or two not post at all and you must then retroclaim (after 28 days), initially by phone. After calling them, they tell you to email the details of the stay to
[email protected] (name, M&B number, property name, checkout date) although I guess you could just skip straight to the email stage. In each case I received a reply within a few hours and the miles were posted by then too.
