I think they guess that we fly that much that we don't need or want one! Getting asked if I am familiar with the seat is annoying.
Having just recently achieved Gold and becoming eligible for the "hello" (my favourite Gold benefit, yes I know how it makes me look...), I've had it on every one of the 5 flights taken so far, save for one where I was fast asleep so they probably just skipped me because of that.
In premium cabins its hard to say if it's the normal cabin welcome or the status welcome, as you get both. In Economy it's obviously pretty easy to tell.
