I'm on the peninsula too and would love to see a dedicated shuttle but I'm not so sure the ridership pencils out. Based on admittedly anecdotal information, not a lot of people get off the train in San Bruno to make the connection to SFO. This may be a chicken and egg thing with the awkward connection but taking Caltrain to SFO or back just doesn't cross most people's mind when they're making their way up to SFO.

I don't think HSR is really designed for airport traffic. I think it's intended to avoid the need for intrastate flights, not to help people get to the airport. However, it may help people avoid short flights, for example someone in the Central Valley could take the train to SFO instead of flying FAT-SFO.
I agree with you on this one but my argument is that because there will be a significant amount of money spent in Millbrae, it seems like an extra BART train set, especially a short one running a shuttle service between SFO and Millbrae, would be up for reconsideration. At least . . . one can hope
