Yes, I had something similar a while ago. Called AA - agent said that she needed to "clean up the reservation" - apparently the old segments were still there even though I could not see them. Once done, I could re-request. May or may not be related - but there are certainly a few issues that can cause anomalies to surface. Best bet is to call when they do.
