1. No one has a physical card and according to website it requires 5 flights in order for them to ship out card.

2. No idea. You should be able to add your Amigo number to a reservation if its option on the website you are booking with. If already booked with UA you will probably have to wait until check-in to apply new Amigo number or call them. I haven't tested this but will in the coming months.
