So to answer my own questions, now based on experience

In general I thought the food ex-SFO and ex-HKG was good by airline standards, but nothing special. Special kudos for the coffee, which didn't taste watered down.

As others have noted, this is easily done simply by informing the FA of your preference. On my flight they were unable to being meal service for 3 hours due to turbulence, at which point all but 3 passengers were asleep.
When I awoke later I was served promptly and the food appeared to be no worse for the wear...

The crab cakes were very good, and the shrimp looked good too. The regular menu items did not appeal at all, so I was very happy with BTC ^

My steak ex-HKG was passable but still overcooked like most airlines - nothing special but certainly edible. Still better than what was shown on regular menu.

The sushi was simply awful, much worse than what I buy in the supermarket (in SFO or SIN). Basically just 6 different pieces of fish, none tasted very fresh. Blech! Next time lamb or lobster.
