Depends. If I am at a place where the decisions are critical to my stay (mostly logistic type details), yes. But the general "tell us your preferences" surveys are, in my experience, rarely actually useful because in many cases the hotel doesn't really follow through (and before everyone jumps in and says "Oh, you haven't stayed at XXXX, where they do" - trust me - I have and they often don't). For me, exceptions have been places like the Mark in NY, luxury ryokans, and one or two others.
Frankly, if a hotel sends one out, expects guests to complete it and then does little to stick to it, they are better off not sending the things out in the first place.
