The comments above imply that it happens with some regularity and that it's deliberate ("fraudulent", "practices", "a lot"). I'd really like to read about it.
Selling First class rides and then claiming that they were upgrades predates the UA=>CO metamorphosis. It happened to me in 2011 but I did not post about it as I refused to take the flight. GA threw quite a fit which was funny.
