Sample doc's note
My boss was recently in an accident and needs to cancel a flight.
I'm in the process of preparing a draft of a letter for the doctor to tweak/sign in order to get the $100 refunded once he's able to fly again and uses the funds from the ticket we're canceling.
I'm not sure what details need to be in the letter. How does this suffice?

I hereby verify that [Passenger full name] suffered a [what kind of injury] as a result of a [occurrence] on [date]. As a result of this injury, I advised [passenger full name] to cancel the trip via air travel that [he/she] had scheduled to [location] from [date] through [date] to [why passenger was going to travel]. It is anticipated that the patient will not receive clearance to fly until at least [DATE].
