It depends on the specific lounge if there is a fee, what the fee is, and what exceptions there are to the fee. You have to look at the "lounge details" of each lounge location page individually, and unfortunately not all of them are clear. For example, the Los Angeles lounge link in a post above says:
Limited to two guests per cardholder - Access may be restricted due to space constraints

Guest Fee - 28.00 USD
Although it's not worded clearly, I'd either assume the worst case (that the $28 fee applies to the first guest), or else contact the lounge for more specifics.

Some lounges make exception for children (but even then the cutoff age varies greatly).

Offhand, looking at various random places, I'm having a hard time finding a lounge that doesn't list a guest fee. So in general, I'd assume a guest fee (in the $30ish range roughly) unless you find there's not.
