I had success with United over a medical emergency. I was booked on an international flight with my kids and I developed a medical emergency. United refunded the change fee for all thereof us, after I submitted medical documents.

I can get supporting documentation, but I'm not sure that United would be very responsive after 4 months. Does any one know of a time limit on this item?
I think you should give it a try. When you change a ticket due to a medical emergency, the change fee is still charged like all other changed tickets. That part is the same for everybody. It is just that a separate group reviews the medical information later and issue a credit to cover the change fees. The two steps are separated steps. You completed step 1 already, and now you are following up with step 2.
