1) They did ask me to name all the countries I'd been to (verbally). The agent looked like she was checking them off the list on the screen. 2) I was required to give my driver's license at the interview. My driver's license had the wrong address but there was no mention that it didn't match what was on the application. I brought along a copy of my mortgage, insurance, and a recent utility bill just to be sure. The agent didn't ask to see them.
The differences in the interview process are amazing. I also interviewed in Detroit at the bridge for Nexus, so I got it from both sides. Neither side asked many questions, and I did not have to recite back what countries I had been through (about 20). The Canadian officer asked if I had ever been in court, and when I said no asked again if I had really never been in a courtroom. I told her I did contest a traffic ticket once (and won), and she moved on to asking what type of travel I did. The whole process, including fingerprinting and watching their short video was propably 20 minutes. I did have to remind them to activate my passport for Global Entry though!

I guess you never know what will trigger them.
