You are correct on both counts. I do believe that there is a fee imposed when downgrading a cabin, though I can't possibly imagine a scenario where one would want to do that.
I had to downgrade from F to J because the flight I was scheduled on (873) had an equipment change and didn't have first class. Obviously, there was no fee when I was downgraded and had the difference in miles redeposited. That would have added insult to injury.
