So Avianca rejected all three of AE's requests to credit this flight. Avianca apparently kept saying my name could not be found in their system - boarding passes, invoice, and itinerary all submitted. AE has manually credited me the status miles for the flight post dated to June. No SE bonus but I'm okay with that to just put it to bed.
More than 3 months after my flights, I too can finally report that AE has manually credited me the requisite status miles for my flights on TACA. In my case, TACA insisted that my fare class was ineligible for mileage earning despite incontrovertible evidence to the contrary (boarding passes, tickets showing the fare class, and screenshots demonstrating the eligibility of that fare class).
I still think it's a little ridiculous that AE requires a 10 week waiting period (when it was very clear that TACA wasn't going to do anything) before finally taking action. Thankfully, I didn't need these miles to cross a status threshold or redeem a flight, but I can imagine that delay being a significant inconvenience for someone who did.
Also, I must say I was quite fortunate that I had an e-ticket that showed the fare class. I know not all online booking engines and/or travel agencies provide that in writing. I'm not sure I would have ever gotten these miles without that piece of evidence.
