Sorry I don't have the card anymore to give you the site... Perhaps another FTer can help you out.

There is no code or anything on the card, just instructions about the choices and the web address (which is interestingly not on a Delta site). Make sure you know the flight number, ticket number, and your PNR so you can complete the form whenever you are able to find it.
