As far as the timeframe for the minimum spend goes, it starts from the time you are approved, not from when you activate it. I know that wasn't exactly what you were asking, but just thought it could be helpful for other people to realize this. Don't want to get caught with having to spend a ton in the last 3 weeks because you thought it started at the activation date (spoken from experience)!
