For what its worth, I asked in the UA forum and this ticketing special requirement seems to be only with AA. I was told that I am able to ticket over the internet and did a dummy booking on UA using TG and it allowed me to book. In this case I am going to use UA miles and avoid this hassle.
Not sure about other airlines but yes, I booked an ex-MNL ticket on CX using BAEC miles pre-Avios and no hullaballoo over paying for the taxes and ticketing in Manila. Paid for everything over the phone with my credit card.
For a 2013 trip, I'm using AA miles for the same flights on CX and had to send someone over to pay for my fees. Hassle but it has to be done.
