I'm certain I didn't click the option to purchase it, and I usually review the charge line-item too (though I can't say for sure this time... no longer remember).
Fortunately, I replied to Allianz' "thank you for purchasing..." email, saying I didn't purchase it, and didn't want it, and they cancelled the policy and reversed the charge. At least it was an easy fix.
