After weeks of seeing just the 40K offer, I tried hooterbif's approach below - and also landed on the 60K offer page. I currently have a Continental card (annual fee due in April so I want to cancel that), and I had a United card several years ago, so I had given up hope on this offer. But since I landed on the page, I took a screen-shot and proceeded with the application. Of course I got "under review - we will let you know by mail in 10 days" - lets see what happens...
And got a call today from Chase. I am holding a dormant Sapphire card (since I switched over to the "Sapphire Preferred" a few months ago) - they approved me for the United card and closed out the dormant card. I also hold a Continental card (which I got April 2011), but they confirmed I will get 50K UA miles on first purchase. So looks like it worked for me...
