They forgot Muse Air, Morris Air, PeoplExpress, New York Air, Texas International, Air New England, Northeast, Air Florida, Presidential Airways, National (the newer all-757 one with the LAS hub), Independence Air, Reno Air, Western Pacific, Skybus, PBA, and those are just the US carriers I can think of off the top of my head.
