Can anyone help with the form (or link) used for visa on arrival at Manas airport (FRU), Bishkek, Kyrgyzstan?

Its a pretty mess at arrival, so rather have it prefilled.

Note to myself. Bring some copies from this visit, for future use.

Tried mailing all Kyrgyz embassies/consulates, half came back as not working.
