Kyrgyz visa on arrival form?
Can anyone help with the form (or link) used for visa on arrival at Manas airport (FRU), Bishkek, Kyrgyzstan?
Its a pretty mess at arrival, so rather have it prefilled.
Note to myself. Bring some copies from this visit, for future use.
Tried mailing all Kyrgyz embassies/consulates, half came back as not working.
One reply, but included only link to embassy site, which of course doestn have that form

I now have a scan of this form. PM if you need one.
Also great trick to have it used as visa to Kazakhstan (southern region, Almaty etc).
Then you need double-entry visa on arrival, but can still fly out from ALA.

You just go to the kiosk at FRU ask for the form and hand over the money; the VOA window was rarely a mess ime.
