Yes, all the miles go into your AA account provided you enter your AA number in the booking.

You can only use eVIPs on 3-class AA metal flights, and only if AA issues the ticket (which looks to be the case if AA is your first carrier, YYZ-JFK.) You can't use eVIPs to upgrade on other carriers.

How are you getting between Shanghai and Nanking? You could do it by going PVG-HKG-NKG (Dragonair) but that would cause you to have five segments in Asia (NRT-HKG-PVG-HKG-NKG-HKG...) which one too many.

Personally I'd choose CX between HKG and NRT and JL between LHR and NRT (mainly to avoid BA fuel surcharges.) BA's seats are parallel to the floor; JL's are slightly tilted. If that matters (doesn't to me) then BA.

Assuming you're returning HKG-YYZ? If that's the case, then you won't have an opportunity to use an eVIP as JFK-MXP is in two-class equipment. You could go JFK-xLHR-MXP using AA to LHR on a 777, in which case you could use the eVIP on that segment, again, only if you bought the ticket through AA.
