Curious if anyone has any ideas what my have happened to my ticket.

I booked a mult-leg journey across Asia and had traveled several legs when I went to Pudong airport to fly to Taipei TSA. I was looking forward to this because I had never flown cross straits directly and never flown into TSA. However when I checked in at Shanghai airlines, I was told that my ticket had been canceled.

A call to corporate TA uncovered that my ticket failed to issue. The TA had no idea why this was or how it happened. Anyone have a clue? Was I supposed to confirm my reservation like back in the paper ticket days? Is this frequent with Shanghai Airlines?

I was lucky enough to catch a later flight into TPE but unlucky enough to be just a dozen people behind Girls Generation in the immigration queue.
