Perhaps there is a promo code that must be entered? There is a space for that -- some of the partners work that way; when you put in the promo code they bring up a way for you to enter your number. I have the same experience as you, and I'm also wondering if there is a promo code, but can't find anything on US Airways website. I've tried a few weblinks off hand such as officemax.com/usairways or officemax.com/dividendmiles and they did not work. I also emailed officemax.com customer service asking about the issue. If I hear back I'll pass it along to FTers.
