Guess next time when I make this reservation again, I will be sure to ask about the coupon...
But the e-receipt says that I rented a Class E and was charged a Class D.
Based on the limited information which you have supplied, you were overcharged. Of course there might be something in your paperwork that which you did not explain, and which indicates otherwise, but again, based on the information supplied, I think that you are owed a refund.
If I were you, I would call Avis customer service and complain. The best customer service agent is James Tucker, and you may reach him by calling 918-624-4216. Explain what was on your original paperwork, and that you were supposed to get a double upgrade, and give him your RA number, and I am guessing that James will process a partial refund to your credit card. Of course no promises, but that is my educated opinion, and what I have done in similar circumstances, and most of those times I have received a refund.
