So if they could just place me on the UG list, why didn't they just do that (as opposed to going through the trouble of re-issuing the ticket)? Is it because they would have to put me on the UG list while the UG window is open (T-5 days)?

It's still odd that as a DL medallion on a DL flight I need to have additional manual processing. Furthermore, the SkyMiles Medallion program T&Cs say nothing about the DL-coded flights requirement.

You are right about earning SkyMiles on AF flights - that has indeed been corrected. Thanks for pointing that out!
