What happens when immigration officer doesn't even open your passport when exiting?

what happens when an immigration officer doesn't even open your passport when exiting the country? I don't care about having no stamp in the passport, but if the passport wasn't even opened, how does the country know you've left(and not overstayed?)

the countries in questions are located on two different continents. (not the US and/or Canada)
