Sorry I didn't post sooner, I just saw this. I run regularly, normally 10km. While you probably CAN run to the Vatican, you'll be taking your life in your hands due to the traffic. That switchback road you see on Google has NO shoulder, no sidewalk, and people drive like, well, Italians!

It was scary. If you're 'sure of foot', if you head right out the gate and then into the hilltop park just down the road, there's a switchbacked path down the front side of that (big) hill, but it's a bit rocky. And no tourist attractions, but shady, cool and pleasant. The other good place to run was along the river (bike path), but you have to get down there first.
I ended up running to the Vatican. The road the shuttle takes is dangerous to walk, so I walked back uphill. The road south (left turn out of the hotel) to the Vatican had no issues. The grade was quite moderate for anyone in decent shape. The major streets were very easy to cross at 7am even if I didn't pay any attention to the red light.
