I'd probably agree with you on the relative accessibility of the major airports - if you've got to cross bridges and tunnels, it can be a problem. If you're in NJ, PA, or even Westchester County (if you go on the Palisades), EWR is probably a lot more convenient. And nowadays EWR has a lot more flights than in the old days, too.
Historically, I haven't had too much problems going to/from JFK, though a lot of that was due to visiting my relatives in Queens, though only on about 2-3 trips to JFK to destinations in Manhattan was it really slow.
Yes, for people in NJ, Philly, PA, and to a certain extend upstate NY, there's really no incentive to go to JFK and risk traffic nightmare.
Going from JFK to Queens isn't that bad, but try to go west into Manhattan and it's a nightmare.
