The worst for me medically has been:

2) Upper throat swap for a genetic TB test (long QTip through nostril)
3) Checking for displaced nose bone x2 (probe in to sinuses)
4) And, at risk of being to personal, varicosele surgery. Thank god for hillbilly heroin!
So far I've never broken anything or had any kind of surgical procedure. In fact, I've never even had a filling done at the dentist because there's never been a need for one — although I expect that to change soon. I've got a tooth I know that needs attention.
