Fees of ~$75-$100 to redeposit miles when an award reservation is cancelled. It is difficult enough to get the awards, so sometimes you need to grab seats far in advance. Then plans change.

When I am booking for my family, this can mean a $400 fee for cancelling "free" tickets which I didn't even use!.
