Undercooked food is major especially if it's chicken. I had undercooked chicken twice in one sitting at an upscale steak and seafood restaurant in San Jose, CA. The chicken was literally raw and on second attempt still raw in the center. The excuse was they don't want to dry out their chicken. I say don't use frozen chicken or defrost properly.
