Elora Gorge falls at the Elora Mill Inn in Elora, Ontario, Canada (f16 / 83s with B+W ND110 filter)
Inglis Falls, Owen Sound, Ontario, Canada (f16 / 80s with B+W ND110 filter)
Hilton Falls Conservation Area located in Campbellville, Ontario, Canada (f16 / 1s with ND8 filter)
Hoggs Falls ~ Grey Highlands, Ontario, Canada (f16 / 77s with B+W ND110 filter)

Ok - going through my online archive, here are a few that I've done...
ISO 200/f6.3 for 5 seconds - not sure of the lens as it's burried in my old collection somewhere...
ISO 100, F22, 30 seconds. Near my office...
ISO 100, F22 1 second - handheld.
ISO 100, F22 13 seconds
ISO160, F11, 40 Seconds or so - Holga.
I'll post some more sometime...

I have done some long-exposure photography with a "Lost America" theme:
And here are some pictures that only involved long exposures, the setting provided the rest:
"Beach on Fire 2008" - An 8 mile long firework in Italy:
