Bummer... I got the one just for plain ol' miles, not EQMs.

This is similar to the promotion run earlier this year in Feb/March, but didnt have the restrictions on which Economy fares you could purchase.

I got nearly the same thing, but instead of 1/2/3 flights, I got it for 3/4/5 flights. I guess they're on to me 
It's interesting that I got the same code: MPD F74. I guess that a promo code alone is not a complete descriptor for a promotion; somehow, your MP number must be involved.

I got a different version with double miles on all qualifying flights after completing one qualifying roundtrip. How do they make these different determinations?

I got the double miles offer and I'm a bit over 43,000 BIS so far this year. Too bad it's so restrictive with fare classes. I don't foresee myself paying a substantial premium to upgrade to F, A, P, C, D, Z, Y, B, E, M, U, H or Q.
