You'll have to get the bus from MXP to Milano Centrale station. As for the rest, here is what I found on tripadvisor:

As far as I know direct trains to Como S. Giovanni leave from Milano Centrale.
Once at Como S. Giovanni it is easier to get a C10 or C20 bus to Moltrasio from outside the station than walk 10 -15 minutes to the ferry point.
I am sorry but I do not know the price of a taxi from Milan to Moltrasio."
Not sure if the bus numbers from San Giovanni are still accurate, so do a google search.
